Our Hospital to a Healthier Home caseworkers are integral in supporting quicker hospital discharge and reducing readmission rates across Wales.

In 2020-21 we worked through the Covid-19 pandemic. In this time, we received 3922 referrals from 17 hospitals across Wales, to help improve patient flow and help keep Wales safe.

We saw an increase in 2021-22, with our service expanding to 18 sites. Every site received more referrals than ever before, and in total hospital teams across Wales referred 5125 patients to the service. 

We are thrilled that Local Health Boards understand the value that H2HH adds to their service provision: we have achieved funding for 2022-23 and look forward to expanding the service over the coming year.
